AFFIDAVIT OF WITNESS AS TO WILL
Before me, the undersigned authority authorized to take acknowledgments and administer oaths, personally appeared:
WITNESS NAME who after being having duly sworn or affirmed to tell the truth, stated:
1. That the instrument on file in this court dated August 23, 2005, is the last will and testament of TESTATOR NAME which WITNESS NAME witnessed.
2. That TESTATOR NAME signed this instrument in their presence.
3. That TESTATOR NAME is well known to WITNESS NAME, and that at the time of execution WITNESS NAME believes TESTATOR NAME to be of lawful age, of sound mind and under no undue influence or constraint.
